  3. Provide Final Product and Input Material details.
  4. Attach supporting documentation:
    • Commercial invoices for all inputs
    • AfCFTA Certificates (if applicable)
    • Proof of purchase for each input material
  5. Use additional forms if more than 10 input items.
  6. Submit completed form to:
